Family Case Management (FCM)
Health Program for Pregnant Women & Children
*Currently all FCM and WIC services at DCHD are being held via telephone.

Family Case Management is a program that assists pregnant women and children to obtain health care services in order to have a healthy pregnancy and promote the healthy development of the child and family.

The goals of the program are:

To provide access to primary health care

To identify and resolve access barriers through referral and linking

To provide health education to all eligible clients

To reduce infant mortality

The Family Case Management Program provides services to families. The Family Case Management program builds close relationships between physicians, hospitals, and other medical providers. The Family Case Management program also teams up with community organizations to discuss obstacles in accessing medical services, child care, transportation, housing, food, mental health needs, and substance abuse services
​
Another function of the Family Case Management program is to provide an authorized agent for completing Medicaid Presumptive Eligibility (MPE) applications for pregnant women and someone to assist families in filling out applications for KidCare insurance assistance.
